Please read booking guidelines below before filling out your form.

• Appointments with Blake can only be made through this website, and in person at the shop. Please do not use social media for booking purposes.

• Our hours are Tuesday-Saturday from 11 am to 7pm. I try to tattoo only within these times so please keep this in mind when entering your availability. I do go past 7 if it is necessary to finish the session or tattoo.

• A $100 Deposit is required to book your appointment. Please do not book unless you are prepared, able, and willing to pay the deposit at time of booking. Do not pay deposit until requested to do so. Deposits can be made by calling the shop, in person, or on our website here. All guidelines regarding bookings and deposits can be viewed here.

• Please allow 1-2 weeks response time.

• You must book your own appointment. If you send me a request trying to book a tattoo for someone else, you will not receive a response. The only exception is for groups coming in together.
